§ 28-2-19. Application; statement identifying independent drivers operating out of licensee's establishment.

Every person desiring to obtain a license required by this division shall:

(1)

File a written application therefor, on forms approved by the consumer affairs department, together with the license fee required by this division. The application shall include the following information:

a.

The name, residence address and telephone number of each individual owner, partner or, if a corporation or an association, each officer and director;

b.

The location of the plant, branch plant, plant store, branch stores and agencies operated under the same trade name as of the date of the application;

c.

The names and addresses and license numbers of each independent driver operating out of such establishment;

d.

Trade names used by the applicant during the previous five (5) years and the locations of prior establishments;

e.

The names and addresses of employers of the applicant during the previous five (5) years;

f.

The trade name to be used by the applicant;

g.

The number of employees, including members of the family;

h.

Any other personal information required by the chief of police to aid in determining the moral character of the applicant and persons associated with the applicant.

File a sworn statement identifying each independent driver operating or intending to operate out of such licensee's establishment.
